Just got this email 5min ago, but the form cannot be view and require permission, anybody received this as well?

Neither did I, maybe they're sending them out in batches. I'll let you know if I get one, and whether it works or not!
 
didn't get that email, but then again it says he opted-in for those types of emails, maybe we just didnt click the checkbox.
 
It looks like a phishing email to me. The from address is fishy as is the awkward English used in the email. I'd delete it and move on if I were you.
