Parental controls are pretty much your only option.

Oh there's also plugins available that block YouTube comments altogether.

With it being an iPad I would use one of the app locking apps where you set which apps need a pin you could do this with the YouTube app. If you wanted to you might be able to do the block from the router and filter traffic to YouTube from the iPad so only that gets blocked its a lot more advanced so I would just keep it simple. Search app lock or app locker on the app store

Make sure the PIN is something only you know and not guessable by your grandson. Do not leave the PIN in a book, a piece of paper, a text file, on your phone, etc.
