New Login Verification?

I've been away from one of my channels for a couple of months. I just logged in again, as I am considering putting up a new video on it later this week. Before I was let into my dashboard, I got hit with this "Verify it's you" screen, giving me several options for doing so.

Has anyone else seen this? Arrgh! They also just forced me to either use my real name or take a G+ page in the name of the channel! I thought they killed that off months ago!

your channel is interesting, i never knew youtube had a start trial button instead of subscribe for free.
anyways if you want to change your google + name i have a tutorial on that its on my channel on my how to playlist title changing your name more than 3times i can explain it here but i feel the video will be easier to understand if any questions let me know

Thanks; but I wanted to keep my channel name as it was, not change it. This was why seeing that was very frustrating; I never wanted to either use my real name, nor link a G+ page to that channel; this box would not go away and gave me no choice except to accept one of the two options of real name or channel name connected to a G+.

One of the channels I have linked in my signature, is a Paid Subscription Channel. There are two channels in the signature; and neither of them is the channel this came up on. That channel is on an entirely different account.
