How to make an end slate for vlogs!

Hey guys! I'm new to vlogging and I need help making an end slate. I want one that shows the previous video, the subscription button, my social networking sites, my website, and stuff like that. Anyone willing to help me out?

I was just going to do it for you. Though if you want me to recommend some software, the best bet would be Photoshop, or you can use GIMP.

use gimp or photoshop to make the actual image then any video editing software with layers for the last video part etc
