How to include written permission of gameplay commentary?

How do you include the address, and what are the exact words to get approved with this information.

Also,

What is the correct category for this:

My video contains only my original content
I have an online license for some or all of content
I have written permission to use some or all of my content

The correct category for this is "I have written permission to use some or all of my content".

Edit; For writing that you have permission, I'd suggest something along the lines of..

Permission to use gameplay by Devolver Digital
[insert link]

And just insert that into your videos description.

I believe most of the process is all robot monitored, probably a group of monkeys knowing Google.

If you get any ID Claims or they won't let you monetize it then just show then that.

I don't think it's considered a written permission .
It's an online license .
I'm not 100% sure though .
Everytime I have to provide the proof I can monetize something , I choose " I have an online license...." .
Then you have to write something down .
Usually I do it this way .
Name of the videogame : Blah blah
Developer : Blah Blah
Publisher : Blah Blah
Permission/Publisher's Video Policy : Link
