Request 5 Second Outro Card!

Hey everyone!
I'm just about to upload a video, and am in desperate need of a quick, simple outro card!
I have one, and the creator is fixing it up, but hasn't gotten back to me.

So if someone could quickly scratch up a blue/green outro card, with the heading HeyloTheo + Sam,
one rectangle for me to put a previous video, and a subscribe button, in the next 10 minutes or so, that'd be
AMAAZING!

I would but it's nearly 1am, and as my channel is shared with my friend, who isn't here with me right now,

A normal outro is. Go to my channel and skip to end end 10 seconds of any video. That is normal outro
