What are the requirements for getting a banner on YouTube channel?

You need to sign up with a partnership network like Fullscreen.net to get that feature. There are many topics on here about joining a partnership.
 
If you means a banner for the layout change, there is no requirements you just have to wait until February 22, when the new layout rolls out for everyone. If you mean for the current layout, Andrew is right. You have to become a partner. Hope this helps.
