Could anyone explain the difference between "custom policy" and "monetized in all countries" please :) ?

Custom policies deal with the claiming settings. The normal default claim most people enable is of course to monetize in all countries. However for example, when you have an artist who licenses their music to certain countries you can have multiple people claiming the video based on location. A custom policy usually refers to examples such as that with certain countries being monetized by certain companies.
 
That isn't a big enough sample size for a CPM average. A simple rounding up or down to the nearest cent can make your CPM jump or dip by a huge amount when it's only a few views. Your 13 cents for 40 views would equal $3.25 per thousand views. If that's gross CPM, you take away 45% for the adsense cut, which leaves you $1.79 (networks would get a percentage of that amount if you are with one), which is well within the average that Frankie stated.
I know nothing of this money stuff :p
I'll take your word for it (which is probably right anyway :D)
