I've seen it a few times. It's a thing that YouTube automatically inserts before videos that have some form of adult language/content...

I believe that's just a pause annotation. I'm not sure how you do them, but they basically pause the video for a few seconds with some custom text.

No, it's not an annotation, it's an actual thing youtube puts at the beginning of a video.

It's a partner rating. You can select whether or not the episode has mature content, and YouTube will place that warning at the front of your videos.

If you are a Partner on Youtube you can create shows!
After that you can create a episode for that show.

So you must label (Does this have Drug Abuse/Strong Language/Mild Voilence etc)
And if you select anyone of them that shall happen.
