Video Piracy through direct googlevideo.com link

Dear all,
I have an interesting video piracy issue: a site embeds my videos using direct link (googlevideo.com) for video streaming, in JWPlayer, adding their own branding and replacing Google Advertisement.
It is a violation of Terms of Service (4A, 4B, 4C, 4D, etc.).
While I would like to report to YouTube this site, I have no idea how to do it.
Perhaps you know, ho to report that kind of violations?

Technical background:
When a viewer enters a page on youtube.com, it shows him all bells and whistles from youtube.com, ytimg.net, etc.
When user begins to watch a video, the video itself is being streamed from a URL like http : // r14---sn-5hn7sn7r. googlevideo . com/videoplayback?{long query stringskipped} .
It is possible to embed that streaming URL in a modified player, like JWPlayer. In that case it is possible to skip YouTube provided Advertisement, and put your own.
It is clearly stated, that this kind of modifications are prohibited by Terms of Use.

If it is your videos, then contact the site that's streaming them and get them to take it down. Contact google and report it.

They are mine.
Site neither want to remove them, nor wants to follow the ToS.
That was my question, actually: where should I write a ToS complain?
2 days of googling gave me no result, so, Im asking the community for help.
 
I'm not sure if it will help, but seeing as google and youtube are essentially the same thing now, you could try copyright@youtube.com and say something like, couldn't find where to send this so if you could forward this to the correct department that would be great.

Will try to email to copyright@youtube.com, but I do not see the way, they can help me. I hope, that they will deal with this issue.
As for the reporting tool - it has no connection to ToS. it is a tool to file a DMCA complain.
