Questions about Model Release Forms

I'm soooo confused about when a Model release form/permission from a place (ie: bar/restaurant/etc) is required to be signed by someone to appear in a video...

Some situations:
1) I'm in a public place (ie: mall, public sidewalk) and just go up to someone ask if they are ok with being interviewed for a youtube video, they say yes. We film, vid is up etc

2) In a public place, person walks through the camera (disrupting the video shot), face is somewhat visible. Would I need a consent for this?

3) At a bar (unsure if private/public) and I just do some general filming of people throughout the bar (ie: walking through a crowd)... we do not show the sign of the bar we are in so its not 100% sure where we are at.


If the videos are not monetized, but you have sponsors paying you to wear something in the video is it considered monetized?

Is there a book/website where I can find all of these answers and more info/situations like this?
