Request One Channel Banner and Avatar

Hey guys, was wondering if anyone could make me a One Channel banner and YouTube Avatar? Im looking for a clean look with my username 'Masuoka101' and 'subscribe to survive' on it. Also a logo that can be used for my avatar. I was wondering if anyone was willing to do this for free aswell??
