How do some Youtubers hear their game sound without using a headset?

I've been watching multiple content creators on Youtube, and some of them have facecams. A prime example: Uberhaxornova, on his streams and even regular gameplay. I know he hears the sound because he'll react to a sound effect or something of the sort. But he doesn't have any headphones on, and you don't hear an echo in the video.
Does anyone know how to get that set-up? Is it just the mic he used or can he somehow sync the audio perfectly so there is no echo?
 
If you look carefully at Nova's videos, you'll see that he's wearing iPhone earbuds.
Yes, but on streams and such with Aleks they aren't wearing headphones, and they can hear the sound still.
If you go on his latest stream for example ( twitch(dot)tv/thecreatures/b/542202764 ) he isn't wearing earbuds.

True. Many streamers use cardioid microphones, which are unidirectional and pick up minimal background noise. They point it directly infront of their face so that they pick up their voice but not the speaker sound.
 
You just turn the volume down. It doesn't need to by high to be heard so the mic wont pick it up. Simple as that. I do it that way.
