Sorry if I have confused you with some of the things I wrote. But I will answer your questions.

1. Yes I can add whatever you want to a Banner or Channel Art.

2. It's $10-$15 each, most people just get the intro alone. And I only work with Intro/Outro Templates (It would take a while to make one from scratch so I won't). So pick a Template you want edited or I can do it for you!
