Copyright Infringement Notification Personal Information

Somebody have re-uploaded almost our entire channel 8 months ago and monetized at least some of the videos. I started to fill the Copyright Infringement Notification form but they ask for my all personal information also there is a statement in read saying:

Claimant information will be published on the YouTube site in place of disabled content.

My question - would be my name and address published on YouTube or just my channel name?
Also how many Copyright Infringement Notification forms I must send to be sure that there will be some action?

I think they will just place your channel name. There is no way there will put your address on a public video.

Just file one complaint per day. After about three complaints, the account will get shut down. Don't waste your time filing a complaint on all of the at once.

In regards to your name showing, it will only show the first field shown below. Put 'chottom' in that field. The rest of it must be your actual information. For 'Authority to make this complaint', I put 'owner'. Your address information that you fill out to the right of this info will not show up online.
