Request Banner, Intro and Outro (Paying)

Okay, so i had some one make me a display picture, no problem. But he never go around to making me a banner.
But I'm now ready to make videos and I feel that I don't look the part. (if you get what I mean.)

So what I need is a banner similar to my display picture and a Intro and Outro for my videos.
I can only spend around $20 or $35 YTtalk cash on this as I am currently having problems with money and its all I can really do with spending.

My channel is going to be mainly gaming and I would quite like an animated intro, but i would settle for any sort of an intro at the moment. And I would also like and outro, which has slots for me to stick annotations to other videos.

But yeah I will leave it to you guys.

I can make super simple intros. It would basically be your channel name in 3d moving around with some pizazz. Let me know if you want one. BTW they're free, as they not advanced. Also, I can't add music to the intro.

My channel name is GSR2013 bur i would like the 2013 to be small compared to the GSR
 
Can you tell me your email? The thing is done, but I need to give you the file, so it doesn't lose quality.
