any Canadians Partnered with Unionforgamers?

I have a channel partnered with them, and am in Canada.
okay great! i actually have a question about the w8 ben form that would need to be filled out when joining Unionforgamers. I've attached a screenshot.

On line number 6, did you leave that space blank or did you fill it out?if you filled it out, what did you put? did you put a SIN number? or a SSN?

and on line number 7, did you leave that blank or did you fill it out? if you did fill it out, what reference number are they referring to?

and lastly, for line number 10, did you leave it blank or did you fill it out? if you did fill it out, what did you put (i would like an example, especially for the part that asks for an explanation of a reason the beneficial owner meets the terms of treaty article.)

I am kind of debating whether or not i should leave the spots blank as UnionForGamers said some parts are often left blank, but i am also worried that if i don't fill out the w8 ben properly, i may run into tax problems during tax time, so thats why i am asking a fellow Canadian UnionForGamers' partner like yourself to hopefully help me out . :)

Here's the instructions from the IRS. http://www.irs.gov/pub/irs-pdf/iw8ben.pdf

Leave 5 blank as you don't have a SSN.
Leave 6 Blank as you don't have a TIN, but fill out the birthdate in line 8
Leave 7 blank unless you have been provided a reference number by a withholding agent (you probably haven't)
Leave 10 blank as you are not an employee of Curse and have no applicable tax treaties covering what you're doing between Canada and the US.

Now, I need to clarify one thing on all of this. I am NOT a tax professional. You probably should consult with one. That said, read through the instructions document and it will confirm what I'm telling you here. US tax laws are unnecessarily complicated.
