I received an email with the following contents "
Dear duy phat tran,
You are receiving this message because all or part of your tax/payment forms are incorrect. Please make sure to read these instructions carefully and make the necessary changes.

- Check the "Individual" box for tax classification
- Provide your current mailing address
- Provide a correct Paypal email address
- Fill out your first and last legal name
- Provide your current address including your house number, street name, city, state/province, country and postal code
- Sign the document with your legal name. Make sure the signature matches the name on the tax form.


Please be sure to double check all your information before re-submitting it as we cannot process payments to you until this information has been accurately filled out and verified.

If you have any questions, please contact your partnership manager or info@makerstudios.com"

Follow the instructions they gave you...? Lol.
 
Considering that your name has more than just first and last name, if you have a social security card pull that out and use the name they have on there, if you are not in the US chances are it is the name that you are somehow getting incorrect, either the order of the words or the spelling
