Still no Form?

It's been about 3 months since I've joined RPM and they still haven't given me the Tax/payment form. I've emailed them about a month ago and I haven't gotten anything back. Could someone please help me?

It's in the dashboard I believe (when I was in RPM, along time ago). There is an email you send them to gain access but I can't remember what it is

When I click on that button on the dashboard it says: "There was an error retrieving your tax and payment information. If the problem persists, please contact us through dashboard feedback."
