PLEASE BE SURE YOU PUT YOUR FULL LEGAL NAME IN
SPACES THAT ASK FOR YOU TO PUT YOUR NAME.

PLEASE FILL THE FORMS OUT CAREFULLY.

Why sid i received something like this again? Ita simmilar to the normal contract

how did you send back your contract? with mail or e-sign. You can also ask them why to sign again.
