Question about RPM network application.

When I try to apply I enter my full name but there is no next button or something to proceed. There is a grey box below and when I click it, it turns blue and does nothing.
Please help me!

How to Fix :
1) X out of browser
2) Never go back to that page or attempt to partner with them again
