HELP! someone copied my channel & they're still there!

Hello so this guy legit copied my channel and re-uploaded 2 of my videos, I've reported the channel before and I reported it again today but I was surprised that it was still up since I've reported it before, what's the best way to contact youtube to get rid of this copy?

thanks! I have a problem though, I don't want the copy-cat to know information about me since they might use it against me, it says in the request form that I need to put down my full name and address...is there another way?
